Your Long Medical Plus health screen has been scheduled as above and will take approximately 2 hours. The link below is to a medical questionnaire which is to be completed and presented to the receptionist on arrival at the clinic.

If you are unable to complete the questionnaire prior to your appointment, please arrive 15 minutes before your appointment time to avoid any delay.

Please remember it is important to arrive promptly for your appointment as late arrival may mean the examination being rescheduled and a cancellation fee being charged. If you wish to reschedule your appointment, you may do so online up to 24 hours before your original appointment time

The purpose of the Health Screen is:

To give advice about lifestyle issues
To try and reduce the risk of future disease
To screen for previously undiagnosed medical conditions
To make recommendations for follow up or treatment as appropriate by a GP or specialist.

It is not designed to deal with specific problems, which are best dealt with by your usual GP.

Please note the following

1. During the Health Screen, a blood sample will be taken for analysis.

2. Any regular medication should be taken as normal and with a little water if required. If you are diabetic, it is important not only that you take your medication as normal, but also eat as normal.

3. If you wear spectacles or contact lenses, please bring these to the appointment with you.

Should you need to cancel or reschedule your appointment you must do so as soon as you can. If you cancel your booking with less than 24 hours notice, we will make a cancellation charge of 100% of the full screen cost.
