Online forms are a direct extension of the medical records system. These allow you to get the patient to complete large parts of medical forms and questionnaires before coming to the practice.
Patients are notified of a form to be filled in by a line in the confirmation email. Clicking on this link activates the form and an event in the client log showing that the patient has opened it. The patient can then complete the form and save it at their convenience. If they have a username and password to the patient portal, they can also save and return to the form at a later date if they need to.
The data saved from the form is inserted directly into the Journal as well as the clinical forms. This saves them a huge amount of time as the data does not have to be retyped.
The system can also use forms to calculate scores based on form responses so the patient could answer a series of simple questions online which would then be interpreted by the system and a risk score calculated.
